Initial Consultation
Your initial appointment will most likely consist of a consultation explaining your diagnosis and treatment options. For more minor procedures, surgery can often be done on the same day as your initial appointment. However, a complex medical history, treatment plan or the desire for IV anesthesia will require an evaluation and a second appointment to provide treatment on another day. Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable
- A list of current medications you are presently taking
- If you have medical and/or dental insurance, please bring the necessary documentation so that we can contact your insurance company to assist you in receiving your maximum insurance benefits. This will save time and allow us to help you process any claims by verification of your benefits.
- If your insurance requires a referral from your Primary Care Physician (PCP) or Dentist, it is your responsibility to provide that referral at the time of your initial consultation.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the time of the consultation visit.
A pre-operative consultation and physical examination is mandatory for patients having IV anesthesia for surgery. Please have nothing to eat or drink six (6) hours prior to surgery. You will need to have an adult escort bring you to the office, remain in the waiting room area during the procedure, and take you home immediately upon discharge.
Please alert the office if you have any medical condition which might be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves or joints, heart murmurs, etc.) or if you are on any medications (i.e. heart medications, aspirin, anti-coagulant therapy, etc.).
X-rays
If your dentist or physician has taken x-rays, you may request that they be forwarded to our office. If there is not enough time to be sent via mail, please pick them up yourself and bring them with you for your appointment. If additional films are necessary, they can be taken at our facility.
